A Resource Page for ACMH Community Health Programs and Partners.
As a hospital deeply rooted in the well-being of our community, ACMH actively fosters programs and partnerships to address critical health needs both in and beyond our walls. We work collaboratively to promote preventive care and reduce health disparities, ensuring support reaches those who need it most. The following programs and partnerships are part of our ongoing commitment to advancing health equity and building a healthier, more resilient community together.
ACMH has partnered with Pennsylvania 211 to help you "Get Connected. Get Help."™
If you need to connect with resources in your community, but don’t know where to look, PA 211 is a great place to start. Nutrition: A Critical Link to Your Health
The ACMH Food Pantry and Care Package Program: Making a difference, one meal at a time... ACMH offers two FREE programs designed to address food insecurity within the ACMH patient population: The FOOD BANK AT ACMH and the CARE PACKAGE PROGRAM.
The Food Pantry at ACMH
The idea behind the ACMH Emergency Food Pantry is that we will bridge the gap for our patients and help to create relationships with organizations that can assist them as long as they need.
The Care Package Program
The mission of the program is to provide nutritious food to patients in need, following their discharge from the hospital. Eligible patients receive three days worth of healthy frozen meals, including specific items for diabetic and cardiac diets.
Armstrong County Community Action Partnership (ACCAP)
The ACCAP vision is to empower every individual and family to achieve lasting independence. They do this by providing direct services to meet their immediate and continuous needs, connecting them to essential resources, as well as offering sustainable solutions. Collaborating with partners, they provide the support that builds a community where self-reliance and prosperity flourish, reducing the need for government aid.
