Stroke Team

The purpose of the ACMH Stroke Team is to ensure consistent, timely and appropriate treatment of all patients admitted to ACMH with a diagnosis of stroke. Patients may be admitted directly to ACMH Hospital or transferred from another facility to the ACMH Skilled or Acute Rehab Unit.

Beginning with the Emergency Room, continuity is maintained across all levels of service including ICU, the Acute Hospital, the Skilled Nursing and Acute Rehab Units, and ultimately through an Outpatient Therapy program.  A Stroke Team consultation results in automatic evaluations by a Neurologist, Physiatrist, Rehab Nurse, PT, OT, SLP, Social Worker and Dietitian.  Twice weekly meetings of the Stroke Team, utilization of the NIH Stroke Scale and notes to the patient's primary physician provide quick response to problems and effective communication between all members of the health care team.

 The physical location of the Rehab Units within the hospital provides an additional measure of security through immediate access to emergency medical care and diagnostic testing.

 Because Armstrong County has been identified as a geographic region with a significantly high incidence of stroke, the availability of comprehensive, compassionate care of stroke victims and their families close to home is of great benefit to the ACMH service population.   

All rooms on the Rehab Unit are private with internet access for patients, and convenient, free parking for family and visitors.
